About The LMA Fastrach #3
The LMA Fastrach (Intubating LMA or ILMA) is a specialized supraglottic device designed for difficult airway management, enabling rapid blind or fiberoptic-guided tracheal intubation. It features a rigid, anatomically curved tube with a handle for manipulation, a guiding ramp, and a dedicated, wire-reinforced endotracheal tube (ETT) for high-success intubation.
Key Aspects of the LMA Fastrach:
- Design: The device has a rigid, anatomically shaped handle for optimal positioning and a bar that lifts the epiglottis, directing the ETT into the trachea.
- Intubation Technique: It allows for blind passage of the ETT (typically size 7.07.5 mm). Success rates are high, reported around 75% on the first attempt and up to 99% with fiberoptic guidance.
- Chandy Maneuver: If intubation fails, the Chandy maneuverlifting the handle slightly to align the device with the glottic openingis used.
- Removal: Once the ETT is placed, the ILMA is removed over the ETT, often using a stabilizer rod to prevent the ETT from being pulled out.
- Indications: It is ideal for patients with restricted neck mobility, suspected difficult airways, or failed direct laryngoscopy.
